normal align=left>二、投標申請人資格要求：
normal align=left>參加競標的投標人必須滿足投標資格要求中的所有條款，并按照相關規(guī)定遞交資格證明文件。
normal align=left>1、投標人必須是中華人民共和國境內(nèi)注冊，具有獨立法人資格；其經(jīng)營范圍應包含：醫(yī)療器械的生產(chǎn)（制造商）或銷售（代理商）。
normal align=left>2、投標設備在國內(nèi)銷售沒有不良記錄、沒有發(fā)生過重大質(zhì)量問題或安全事故；
normal align=left>3、投標人必須是所投產(chǎn)品的制造商或代理商。
normal align=left>4、本項目不接受同一品牌的兩個及以上投標人同時投標，開標后，若出現(xiàn)同一品牌的投標人有兩家以上，那么對同一品牌的所有投標人予以廢標。
normal align=left>5、投標貨物必須符合國家標準、行業(yè)標準和專業(yè)標準等相關標準，并符合中華人民共和國國務院令第650號修訂后的《醫(yī)療器械監(jiān)督管理條例》相關規(guī)定。
normal align=left>6、生產(chǎn)企業(yè)須具有醫(yī)療器械生產(chǎn)許可證；代理商（經(jīng)銷商）須具有醫(yī)療器械經(jīng)營許可證。
normal align=left>7、具有良好的商業(yè)信譽和完善的售后服務體系，并能承擔采購項目供貨能力和服務。
normal align=left>8、企業(yè)注冊地或項目所在地檢察機關出具的無行賄犯罪記錄證明。
normal align=left>9、投標人必須滿足對投標資格的其他要求。
normal align=left>10、實質(zhì)響應招標文件提出的關鍵技術參數(shù)等要求。
normal align=left>11、本項目不接受聯(lián)合體投標。
normal align=left>三、招標文件的獲取：
normal align=left> 本次招標項目取消現(xiàn)場報名，凡符合上述要求的投標人請于2017年1月18日至2017年1 月 23 日登陸安陽市公共資源交易中心網(wǎng)站（www.ayggzy.cn），完成用戶注冊后下載招標文件。
normal align=left>四、請投標人注意：
normal align=left>各潛在投標人請到安陽市公共資源交易中心辦理數(shù)字證書認證，辦理及使用流程請到安陽市公共資源交易中心網(wǎng)站資料下載欄中下載（醫(yī)療設備系統(tǒng)使用說明—>醫(yī)療設備網(wǎng)上投標系統(tǒng)操作文檔）。
normal align=left>辦理地址：市民之家二樓西廳31號窗口。請在工作時間內(nèi)辦理數(shù)字證書，以免影響正常投標。工作時間：上午9：00—12：00，下午 1：00—5：00
normal align=left>咨詢電話：0372-5116081數(shù)字證書需要現(xiàn)場辦理，請將各項材料準備齊全。
normal align=left>招標文件售價：300元/份，售后不退。開標時交納。
normal align=left>五、投標文件的上傳及開標時間
normal align=left>5.1投標文件上傳的截止時間及開標時間為2017年 2 月 14 日 上午10:00整地點為安陽市民之家四樓公共資源交易中心第 五 開標室。
normal align=left>5.2 開標截止時間前投標人須將電子投標文件上傳完畢，否則招標人不予受理。
normal align=left>5.3電子投標文件遞交方式：
normal align=left>網(wǎng)上遞交：投標人必須在辦理數(shù)字證書后在投標截止時間前完成所有投標文件的上傳，逾期將無法上傳電子文件。
normal align=left>六、招標公告發(fā)布地址：
normal align=left>本招標公告同時在《中國采購與招標網(wǎng)》、《河南省政府采購網(wǎng)》、《河南招標采購綜合網(wǎng)》、《安陽市政府網(wǎng)》、《安陽市公共資源交易中心》網(wǎng)站上發(fā)布。
normal align=left>七、注意事項
normal align=left>7.1 投標人獲取文件后，如對招標文件內(nèi)容有質(zhì)疑應在2017年 2 月 4 日下午16:30 前以不記名形式上傳至安陽市公共資源交易中心網(wǎng)站。
normal align=left>7.2本次招標項目如有變更或延期，投標人均可在安陽市公共資源交易中心網(wǎng)站直接下載補充文件，投標人應隨時關注安陽市公共資源交易中心網(wǎng)站。
normal align=left>7.3開標后由評標委員會對投標單位的資格證明材料進行資格審核，不符合項目資格條件的投標單位的投標將被拒絕，投標單位應自負風險費用；提供虛假材料的將進一步追究其責任。
normal align=left>7.4特別說明：電子投標文件通過網(wǎng)絡上傳成功即視為簽到成功，投標文件因網(wǎng)絡原因無法正常上傳，或電子文件格式不正確無法打開上傳文件的，視為無效標，現(xiàn)場不接受紙質(zhì)版投標文件，同時現(xiàn)場不接收u盤上傳標書文件。
